Home to Decide is a unique Hilton care home avoidance service. Most people would prefer to live in their own home rather than a care home, but the time can come when the challenges of living at home become greater. It is never easy to decide when these challenges become so great that it is no longer safe to live at home. Home to Decide provides the information to make that decision.
The service developed from our belief that many people were leaving their own home to enter residential care before they needed to. Sometimes this happens because the challenges of living at home appear to be insurmountable, sometimes it is due to a period of illness or an infection and sometimes it can be due to an attempt to eliminate risk rather than manage it.
To date, an amazing 67% of people completing the service have been able to continue to live at home.
The service lasts up to 14 days starting at an intensive level and reducing in line with the individual’s progress.
Sometimes factors including mobility, health conditions and cognitive impairment can make staying at home challenging. Hilton Nursing Partners want to help meet that challenge enabling people to continue to live in their own home. Home to Decide is for people who would prefer to live in their own home but are facing some challenges to ensure that they can continue to live independently, safely and in comfort.
This unique service involves working in partnership with individuals to optimise their physical and emotional wellbeing, review their physical surroundings and to identify what support is needed and when. Home to Decide supports people to live independently and safely in their own home, where they want to be.
Home to Decide is a unique service which recognises that leaving your own home to live in a care home is a major decision. Our focussed service is based around each individual’s aims and aspirations. We work in partnership with each individual to optimise their independence. We provide an expected outcome after 5 days, and update this during the remainder of the service. We produce a detailed report at the end of the service.
Home to Decide starts at an intensive level of up to 24 hours per day in the first three days, then reduces in line with the individual’s abilities. At the end of the service we will have identified the best option going forward. Where a care home is the best option we will provide support in identifying the care home that suits you best.
Home to Decide has successfully kept 67% of people, who would otherwise have gone in to residential care, at home. The service will explore all options to keep you safely at home, and to maximise your independence, and provides reassurance that you or your loved one are making the right choice.
